Our Client, Manchester City Council, is seeking an Adult’s Social Worker to join their Transitions Team.

Responsibilities of this role:
- Assess young people’s needs as they move from children’s to adult services
- Plan and coordinate smooth transitions (Care Act–led)
- Work with families and multi-agency partners
- Support independence, choice, and outcomes
- Safeguard vulnerable young adults
- Complete care/support plans and reviews
- Ensure legal compliance (Care Act, MCA)
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
- Must have minimum of 3 years experience working as a Qualified social worker
