Our Client, Manchester City Council, is seeking an Adult’s Social Worker to join their Transitions Team.

This is a Hybrid Working Role!

Responsibilities of this role:

• Assess young people’s needs as they move from children’s to adult services
• Plan and coordinate smooth transitions (Care Act–led)
• Work with families and multi-agency partners
• Support independence, choice, and outcomes
• Safeguard vulnerable young adults
• Complete care/support plans and reviews
• Ensure legal compliance (Care Act, MCA)

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
• Must have minimum of 3 years experience working as a Qualified social worker
