This ACP Geriatric Social Worker provides multiple social work services including: community-based case management for frail, isolated older adults, elder care consultations either in client’s home or the ACP office and response to calls on the Information and Assistance telephone line and/or requests for information via email.
This is a full-time, exempt position, located at our main headquarters (please be aware that our headquarters will be temporarily located at the Cubberley Community Center, 4000 Middlefield Road, during a major remodel of our downtown facility, with an expected return date of Spring, 2019.) Our work schedule is Monday through Friday, from 8:30am-5:00pm. Some local travel, via your own vehicle, is required.
This position includes significant career opportunities for the incumbent to develop areas of expertise in public speaking engagements and community based educational programs. The ACP staff often speak at, and help plan, Avenidas’ annual housing, caregiver and other conferences and workshops.
Responsibilities and Tasks
The Geriatric Social Worker will:
Conduct both brief and comprehensive geriatric psychosocial assessments of clients’ needs (in the office or during a home visit) for emotional and social support, community resources, information, guidance, intervention and client advocacy. The Geriatric Social Worker will act as an advocate in locating transportation, housing, home care, legal and financial aid resources.
Provide screening and intake by telephone regarding client’s concerns, requests for information, elder care consultation or case management. This may involve providing information on area resources for housing, legal aide, transportation, home care and financial assistance or referral to one of many Avenidas programs, including the Rose Kleiner adult day health program, Door to Door transportation, handyman, in-home tech support and/or membership in the Avenidas Village.
Provide brief community- based case management services per county and city grants, to link clients to needed services, provide follow up and advocacy on behalf of older adults in need of resources.
Present client case work to supervisor during weekly supervisory sessions.
Coordinate case management evaluation surveys for grant requirements.
Update community resource files, identify new services, maintain accurate records and files.
Maintain accurate records of client contacts through chart documentation and enter statistical data into computer database.
Become knowledgeable of and develop working relationships with local elder care providers: this is always an on-going process.
Occasionally perform certain social work tasks at the Rose Kleiner Adult Day Health Center
Other related duties include billing for grant based county and city case management and consultation monthly services our on-line data entry program. Training provided.
Education and Experience:
Experience conducting and documenting psychosocial assessments and all client casework through to case closure.
Hands on experience providing computer data entry, tracking of confidential and statistical information, completion of billing and administrative tasks as assigned.
Prior experience in working with elderly clients, particularly through case management services, required. Strong preference for experience working with intergenerational family and social support teams and with professional elder care medical, legal, financial and custodial care providers.
Familiarity with county and city area wide agencies on aging and their resources, including contacting Adult Protective Services preferred.
Knowledge, Abilities and Skills: Knowledge of and capacity to utilize community resources; ability to make psychosocial assessments and develop and implement viable person-centered care plans; demonstrated counseling ability; excellent interpersonal, listening, oral and written communication skills; ability to keep statistical and narrative records; exercise independent judgment; sensitivity, maturity.
Valid California driver’s license, automobile and automobile insurance. Bilingual preferred: Mandarin or Spanish.
