Job Description


The Orchards at Bartley Healthcare is currently hiring a PT RN Wellness Nurse Supervisor. Looking for someone who is flexible willing to do off hours and weekends if needed.

Make written and oral reports to the Wellness Director and/or Executive Director concerning the operation of the Wellness Department.

Develop methods of coordination of health services with other resident services to ensure the continuity of care.

Participate in The Orchards surveys by authorized governmental agencies.

Responsible for verifying all medical form completion prior to resident’s entrance.

Responsible for advising resident(s) when annual physical examination is due.

Continually assesses residents to verify their appropriateness to live at The Orchards.

Performs administrative paperwork as needed.

Administers Mantoux and Hepatitis B vaccinations and any other appropriate vaccination.

Act on behalf of the Wellness Director in his/her absence.

Maintains a liaison with residents and family members.

Promotes exceptional customer service at all times.

Makes routine inspections of The Orchards to make certain policies and procedures are being implemented.

Meets with each new resident to familiarize them with your job functions and duties.

Assists with medication administration, as needed.

Work in conjunction with resident’(s) attending Physician to make certain highest quality of services is being provided.

Make written and oral reports/recommendations to the Wellness Director, VP of Operations or Executive Director, concerning nursing procedures.

Assure that the office is maintained in a clean and safe manner and that necessary equipment and supplies are maintained to perform required duties.

Assist in standardizing the methods in which work will be accomplished.

Perform administrative requirements such completing necessary forms, reports, etc., and submit to Wellness Director and/or the Executive Director as required.

Assist in administrative studies and projects assigned or that may become necessary.

Participate in and help administer Performance Improvement Programs that include facility inspection and employee in-service monitoring.

Other related duties and responsibilities that may become necessary or as directed by the Wellness Director, VP of Operations, Executive Director

Coordinate and help develop Cedar Spring Program (the memory support program).

Develop, implement and maintain a procedure for reporting hazardous conditions or equipment.

Attend and participate in continuing educational programs to keep you abreast of changes in your profession, as well as to maintain current license.

Committee Functions

Provide written and/or oral reports of the Wellness programs and activities as required, or as may be directed with such committees.

Serve on, participate, and attend various committees of the facility, (eg. Infection Control, Budget, QI, etc) and as appointed by the Wellness Director and/or Executive Director.

Serve on various committees of the facility as directed by the Wellness Director, VP of Operations , Executive Director

Evaluate and implement recommendations from established committees as they may pertain to nursing services.


Personnel Functions

Determine the staffing needs of the Nursing Department necessary to meet the total nursing needs of the residents.

Recommend the number and level of nursing personnel to be employed.

Assist the Wellness Director in the recruitment and selection of nursing personnel.

Assign a sufficient number of licensed practical and/or registered nurses for each tour of duty to ensure that quality care is maintained.

Assign a sufficient number of nursing assistants for each tour of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.

Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks.

Delegate to nursing personnel the administrative authority, responsibility and accountability necessary to perform their assigned duties.

Develop, maintain and periodically update the written procedure for ensuring that professional nursing personnel, including private duty nurses, have valid and current licenses as required by this State.

Make daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards.

Monitor absenteeism to ensure that an adequate number of nursing personnel are on duty at all times.

Review complaints and grievances made or filed by department personnel.

Make appropriate reports to the Wellness Director and/or Executive Director as required or as may be necessary, including department performance evaluations.

Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status.

Staff Education

Ensure that all department staff members receive mandatory inservice training as required by the federal, state and local standards, guidelines and regulations as well as the facility’s guidelines.


Communicate effectively with Department staff to ensure maximum efficiency of the department. All team members are aware of important issues and receive information timely. The sharing of information is encouraged daily.

Communicate effectively with other managers to include daily events, scheduling conflicts, requests, changes, admissions, discharges, resident and family concerns, and other pertinent information

Communicate effectively in outlook, memorandum and verbal formats
