Hospital Discharge Social worker based in Hackney!

The job will involve the usual frontline tasks, including:

Completing Care Act Assessments
Conducting Care Act Reviews
Performing Safeguarding and Human Rights Assessments
Completing CHC Checklists
Collaborating with hospitals, families, and other agenciesRequirements as a Social Worker include:

SWE Registration
Eligible to work in the UK
Drive and have access to a car
Post-qualified experience
