Adult Social Worker - Disabilities Team (Lincolnshire, £38/hr)

Are you a skilled and compassionate Social Worker with experience in supporting adults with disabilities? Lincolnshire County Council is looking for a committed Adult Social Worker to join their Disabilities Team, making a real difference in the lives of adults with complex needs.

🔹 Role: Adult Social Worker - Disabilities Team
🔹 Location: Lincolnshire
🔹 Rate: £38 per hour
🔹 Contract: Full-time | Temporary (initial 3-6 months, with potential to extend)
🔹 Start Date: ASAP

🔍 About the Role:

You'll be working with adults with learning disabilities and/or physical disabilities to assess needs, promote independence, and ensure access to the right support and services. The role requires a strengths-based, person-centred approach in line with the Care Act 2014 and relevant legislation.

✅ Key Responsibilities:

Complete Care Act assessments, support plans, and reviews

Coordinate care packages and liaise with providers and health professionals

Carry out Mental Capacity Assessments and manage safeguarding concerns

Promote independence and community-based solutions

Maintain accurate, timely records and reports in line with statutory duties

🎓 Requirements:

Qualified Social Worker with Social Work England registration

Experience in adult social care, ideally in a disabilities team

Strong working knowledge of the Care Act, MCA, and safeguarding procedures

Excellent assessment, planning, and communication skills

Ability to work independently and within a multidisciplinary team
