Leaving Care Personal Advisor

Department: Children's Services

Work Schedule: 7.4 hours per day, 5 days per week.

Location: Manchester, UK.

Driving Requirements: Must have valid driver’s license and own vehicle.

Purpose of the Job:

  • Commitment: Deliver outstanding services to care leavers with energy and enthusiasm.
  • Support: Provide interventions to young people aged 16-25, helping them transition from care to independence.
  • Assessment: Conduct assessments of need post-18 and provide practical and emotional support based on co-developed pathway plans.
  • Commitment: There is a real commitment and passion in BwD to deliver outstanding services to care leavers.
  • Team Collaboration: Work alongside a diverse and cohesive team providing support to young people/young adults.
  • Support Services: Provide support and interventions to young people in, or leaving care aged 16-25 years.
  • Assessment: Carry out assessments of need post-18 and provide aspirational, practical, and emotional support based on co-developed pathway plans.

Knowledge, Skills & Abilities:

  • Qualifications: NVQ level 3 in child care/social care or equivalent relevant qualification, or degree level.
  • Legislation: Understanding of Leaving Care and Children legislation and guidance.
  • Research: Familiarity with key findings from evidence-based research, Serious Case Reviews, and inquiries related to Child Exploitation.
  • Experience: Minimum 1 year of working with young people up to 25 years within clear boundaries.
  • Assessment: Knowledge of formal assessment processes and ability to formulate, implement, and review individual pathway plans.
  • Safeguarding: Knowledge of adult safeguarding and relevant legislation related to young people leaving care.
  • Trauma: Understanding of childhood trauma and adverse childhood experiences.
  • Exploitation: Experience working with young people vulnerable to exploitation, gang involvement, drug and alcohol misuse, or criminal activity.

Main Areas of Responsibility:

  • Relationship Building: Develop positive, meaningful relationships with young people/adults.
  • Independent Living: Equip young people with skills to manage money, find accommodation, access education, training, and employment, live healthily, and maintain social networks.
  • Service Development: Participate in the ongoing development of the service, ensuring compliance with legislation and guidance.
  • Multi-Agency Work: Collaborate with other agencies to engage young people in education, employment, and training opportunities.
  • Accommodation: Work with housing providers to ensure suitable accommodation for care leavers.
  • Best Practices: Promote initiatives and good practice models from the National Leaving Care Benchmarking Forum.
  • Voice of Young People: Ensure young people's voices are heard through participation and co-production.
  • Case Management: Maintain high standards of case recording, monitoring, and administrative records.
  • Council Policies: Adhere to Council policies and procedures, including health and safety, equal opportunities, customer care, and strategic objectives.
  • Training: Participate in in-house and inter-agency training
