Family Support Practitioner

Location: Hammersmith, London (Hammersmith & Fulham Council)
Contract: Initial 3 months
Hours: 36 hours per week
Pay: £27 per hour (via umbrella company) (£21 PAYE an hour)

Are you an experienced Early Help or Family Support Worker with a strong background in delivering early intervention services? Hammersmith & Fulham Council is seeking a passionate and dedicated Family Support Practitioner to join their multi-agency team in Hammersmith.

Key Responsibilities:
Collaborate with professionals from CAMHS, schools, school nursing teams, police, health services, and the voluntary sector to deliver coordinated and consistent support to vulnerable families.
Deliver 1:1 support plans, establish baselines, agree outcomes with families, and review progress regularly to ensure positive and sustained change.
Ensure the voices of children, young people, and parents/guardians are consistently sought, heard, and appropriately represented.
Record feedback, track family outcomes, and provide evidence of impact to line managers.
Maintain timely and accurate case notes and service plans on local authority systems, in line with information governance policies.Essential Requirements:
Proven experience as an Early Help or Family Support Worker, with a track record of managing and progressing a caseload independently.
Demonstrated experience completing Early Help Assessments and facilitating Team Around the Family (TAF) meetings.This is a fantastic opportunity to make a meaningful difference in the lives of families and children, while working in a dynamic and supportive multi-agency environment.
