Your new roleWe are seeking a dedicated Targeted Support Worker to join a dynamic team delivering housing-related support across Mendip, with a focus on Frome and Shepton Mallet. This is a full-time role (37.5 hours per week) on a fixed-term contract until 31st March 2026, offering a competitive salary of £25,235 per annum. You'll work within a flexible rostered system between 08:00 and 20:00, primarily Monday to Friday, with occasional weekend and out-of-hours on-call responsibilities. You'll manage a caseload of up to 10 clients, providing tailored support to individuals with complex needs and histories of rough sleeping. The role is mobile, requiring a valid UK driving licence and access to a vehicle for business use.Responsibilities include - Conduct needs assessments and develop personalised support plans.- Help clients maintain tenancies and access essential services.- Provide budgeting advice and support income maximisation.- Promote independence, self-esteem, and social integration.- Liaise with external agencies and family/support networks.- Maintain accurate records and produce quarterly reports.- Source furnishings and essentials through grants and charities.What you'll need to succeedTo thrive in this role, you'll need to be empathetic, resilient, and confident working with individuals facing complex challenges. You'll bring a proactive, person-centred approach and the ability to build trust and rapport with clients from diverse backgrounds.
