This is a flexible, largely remote role, with a focus on delivering high-quality statutory assessments and support planning across designated geographical areas within Cornwall.Work will be allocated based on locality rather than traditional mid/east/west divisions, allowing for more efficient and manageable caseloads.Key Responsibilities

• Undertake Care Act Assessments in line with statutory duties
• Complete Care and Support Planning to meet individual needs
• Carry out Mental Capacity Assessments and associated decision-making processes
• Manage caseloads independently while meeting strict deadlines
• Conduct face-to-face visits where required across Cornwall
• Maintain accurate and timely case recordings

Essential Skills & Experience

• Strong experience in:
  • Care Act assessments
  • Care and support planning
  • Mental Capacity Act and associated assessments
• Ability to work autonomously and manage workload effectively
• Proven ability to meet tight deadlines
• Excellent assessment, analytical, and report-writing skills

Requirements

• Must have access to a car and be willing to travel within Cornwall
• Must live within a reasonable travelling distance to complete visits when required
• Flexibility to balance remote working with fieldwork
