GSTT Community Neighbourhood Nurse (based on Buurtzorg model)

Be able to have the time to sit down with your patients and have valued conversations, have time to listen and provide proper holisitic care. We are offering you the opportunity to work in a way you really want to.

We are seeking dynamic Nurses preferably with community experience to join us here at Guy's and St Thomas'. We have led the way in testing new community models of nursing and are now the first trust in the UK to apply the Dutch Buurtzorg approach to community nursing.

We are now able to work in this new model and are currently rolling out across all community, this is a fantastic opportunity for you to help shape community nursing for the future and to be part of working in this brand new way of working in community nursing in the UK.

We are looking for nurses who work well in a team, can think outside the box, loves going the extra mile, thrive on a challenge and are open to Buurtzorg's approach in the UK Community Nursing field.

This is a fantastic opportunity particularly for current District Nurses wanting to work in a different way where they get more time with their patients in a more holistic manner or people with a range of experience looking to come into community.

The benefit to the patient with this new approach is that the team in place identifies solutions quicker for the patient, improves independence and streamlines care more effectively. This personalized attention and team approach allows individuals to stay in their homes and communities for as long as possible, avoiding hospital admission.

The benefit to the staff member is the professional freedom and responsibility that comes with supervising and supporting each other in self managed teams. Coaches not managers support the team and the Nurses will decide how the care will be provided to their patients. This encourages flexibility and improves staff satisfaction.
We have a structured career progression framework up to band 7 and you will receive training on working within this model from a highly specialised team.

We are currently converting all our district nurse service over to the Neighbourhood Nursing model and you will be part of this exciting change

Please Note : You will move over into the role at your current NHS spine point on Agenda for Change
We work a seven day service between 8.00 and 20.00 with the team arranging the shifts between them, there is an expectation you will work weekends and will work no more than 2 weekends out of 4
