Person Centered Support Specialist, Developmental Disabilities

ABBOTT HOUSE, an innovative community-based social service organization, is seeking a Person Centered Support Specialist for our Services for People with Developmental Disabilities (SPDD) program.


+ BA/BS preferred. Minimum AA degree with experience required.

+ 3 years working with individuals who are developmentally disabled plus knowledge of NYS OPWDD regulations is required.

+ Candidate must be proficient in the use of a computer and appropriate software including Word and Excel.


Responsible to utilize person centered planning tools in conjunction with the OPWDD Agency Protocol Manual to assist people supported by Abbott House in identifying meaningful life goals and activities. Works with the Compliance Standards Manager to ensure compliance with all applicable regulatory guidance and agency requirements. The Person Centered Support Specialist is also responsible for training and providing guidance to all Abbott House staff in learning person centered practices as well as other Agency policies and procedures required to provide safe, ethical and quality services to the individuals supported at Abbott House.

Duties & Responsibilities:

+ Participates in the Council on Quality Leadership's (CQL's) 4-day Personal Outcome Measures (POMs) Interviewer training.

+ Completes Personal Outcome Measures (POM) interviews and other person centered methods with people served.

+ Assists in the scheduling and facilitation of POM interviews for POM trained staff.

+ Completes person centered audits.

+ Analyzes data from POM interviews to identify agency and program specific trends.

+ Presents trend reports to agency leadership.

+ Documents results from person centered planning tools and assists team with supporting the person in the development of goals.

+ Utilizes data and results to help facilitate, encourage and support staff in implementing person centered support plans.

+ Provide training to staff on person centered practices and approaches.

+ Participates in agency wide committees as needed.

+ Attends person centered planning meetings and team meetings as needed.

+ Maintains the highest level of confidentiality.

+ Attends OPWDD trainings as needed.

+ Performs other duties as assigned or directed.

Physical Demands and Working Conditions:

Work may involve the ability to transfer, and lift individuals and equipment of approximately 40 pounds. Work may include continuous walking or standing, manual dexterity, visual and auditory acuity to communicate verbally with people we support, bending stooping and overhead lifting.

Work involves regular contact with persons receiving support. Work may be performed under pressure and may be mentally and physically fatiguing.

This position requires travel. Use of personal vehicle to travel to Abbott House sites and elsewhere in performing job duties will be necessary. Employees in this position must maintain liability insurance on their personal vehicle. Mileage reimbursement for use of the personal vehicle will be available pursuant to Abbott House policy.
