Older People's Mental Health Practice Lead Social Worker

Location: Slough

Post Pay Level: 8 - £44,428 to £49,498 plus £1039 Local Weighting Allowance per annum

Reports to: Service Manager

DBS Requirement: Yes

Main Purpose of Job:

As the OPMH Practice Lead, you will provide professional leadership and support to a team of qualified and unqualified staff within the Older People's Mental Health Service. Your role is crucial in delivering the key strategic aims of the organization, ensuring high-quality outcomes for service users through effective practice and case management of complex caseloads.

Main Accountabilities:

  • Undertake social work on allocated cases, producing outcome-based plans for adults, their carers, and families.
  • Demonstrate expert and effective practice in complex situations.
  • Manage a complex case load and offer expert opinions within the organization and to others.
  • Chair meetings, offer support to case conferences, and produce high-quality assessments and reports.
  • Ensure ongoing work, support plans, and care packages are reviewed in accordance with policies and contractual agreements.
  • Conduct investigations and prepare reports for panels, case conferences, and senior managers.
  • Participate in routine auditing activity to secure the highest possible standards of social work practice and record keeping.
  • Identify targets for staff, monitor performance, and take corrective action where necessary.
  • Develop and maintain effective working relationships with partner agencies to ensure services are provided in accordance with assessed need.
  • Maintain accurate records using the RIO health record system.
  • Liaise with finance and benefits teams, ensuring compliance with the charging process.
  • Contribute to the development of services for older people with mental health problems.

Experience:

  • Significant experience of working with older people with mental health conditions and families in a community setting: Essential
  • Experience of carrying out personalized assessments, support planning, commissioning of care services, monitoring, and reviewing: Essential
  • Experience of working with a range of partners, both statutory and within the community, and ensuring effective use of resources: Essential
  • Experience of undertaking risk assessments and developing risk management plans: Essential
  • Significant experience of supervision, from the point of qualification onwards, to reflect on practice, assessment, and care planning: Essential

Skills and Abilities:

  • Excellent analytical skills, casework assessment, interventions, and skills to develop outcome-based plans: Essential
  • Working within an MDT setting with health and social care priorities: Essential
  • Excellent verbal, written, and IT skills: Essential
  • Communicating clearly and effectively with vulnerable customers in a non-judgmental, non-discriminatory way: Essential
  • Ability to undertake complex social work with vulnerable older people in the community: Essential
  • Maintain clear and effective caseload reporting, complete risk assessments, and care plans: Essential
  • Ability to work on own initiative and exercise judgment: Essential
  • Experience of balancing complex workloads and setting priorities: Essential
  • Demonstrable ability to work effectively and contribute to the management of the team: Essential
  • Work within a changing and complex environment providing services to vulnerable people: Essential
  • High standards of personal and professional conduct: Essential
  • Commitment to continuous professional development: Essential
  • Active evidence of working in a person-centered approach: Essential (

Knowledge and Understanding:

  • Understanding of the statutory framework relating to the assessment of need and provision of services for Older People with Mental Health: Essential
  • Appreciation of the significance of IT and information systems to social care: Essential
  • Understanding and commitment to equality and diversity: Essential
  • Equal treatment and anti-discriminative policies and practices working in ethnically and culturally diverse communities: Essential
  • Knowledge and understanding of local resource and community networks and how these fit into modern social work practice: Essential

Qualifications:

  • Recognized Social Work qualification approved by Health & Care Professions Council (HCPC): Essential
  • Current valid registration with HCPC: Essential
  • Driving License with access to a car on a daily basis: Essential
