Are you a dedicated and experienced Community Psychiatric Nurse (CPN) looking for a rewarding role in Bromley? We have an exciting opportunity for a Band 6 CPN to join our dynamic mental health team, providing high-quality care and support to our community.

Key Responsibilities:

• Conduct comprehensive assessments of individuals with mental health issues.
• Develop, implement, and evaluate personalized care plans.
• Provide therapeutic interventions and crisis management.
• Collaborate with multidisciplinary teams to ensure integrated care.
• Liaise with patients, families, and external agencies to coordinate care and support.
• Maintain accurate and up-to-date clinical records.

Requirements:

• Registered Nurse (RN) with valid NMC registration.
• Significant experience in community mental health settings.
• Strong clinical assessment and intervention skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a team.
• Commitment to providing patient-centered care.
