An opportunity has arisen for an experienced and dedicated Personal Advisor to join a dynamic Leaving Care Service, supporting young people as they transition into independence. This role offers hybrid working, a flexible approach to hours, and the chance to make a genuine impact on the lives of vulnerable young people.

Manage a caseload of care leavers, providing tailored guidance and support in accordance with legislation, policies and departmental standards.

Work collaboratively with partner agencies and professionals to assess needs, plan support and deliver effective interventions.

Visit young people in the community, attend appointments with them, and support them in developing independence skills.

Maintain accurate, confidential records using both manual and electronic systems in line with policy.

Attend and contribute to reviews, meetings and case discussions, including those within external settings such as hospitals, custodial units and local councils.

Assess financial needs, assist with budgeting and, where necessary, handle cash safely and appropriately.

Be vigilant to safeguarding concerns, ensuring young people's welfare is prioritised and appropriate actions are taken.

Plan, conduct and record Pathway Plans, assessments and reviews as required.

Participate in supervision, training, professional development and practice observations.

Ensure all services delivered reflect principles of diversity, inclusion and social justice.
