Key Responsibilities:

*

Complete Care Act Assessments for young people transitioning to adult services.

*

Develop and implement personalised Support Plans.

*

Arrange, monitor, and review support packages to ensure positive outcomes.

*

Undertake safeguarding enquiries in line with statutory duties.

*

Work in partnership with Children's Services, Education, Health, and other stakeholders.

*

Promote independence and wellbeing through strengths-based practice.

*

Maintain accurate case records and ensure compliance with legislation and policies.

Essential Requirements:

*

SWE registered with a recognised Social Work qualification (CQSW, DipSW, or equivalent).

*

Previous experience within Transitions, Adult Services, or Children's Services.

*

Strong knowledge of the Care Act 2014, safeguarding frameworks, and statutory responsibilities.

*

Ability to work collaboratively with multi-agency partners.

*

Right to work in the UK.

Location and Working Pattern:
Based within the Royal Borough of Kingston upon Thames Transitions Team, with hybrid working available depending on service needs.
