Role Overview:

* Swansea are seeking a skilled Band 6 Locum Occupational Therapist to join their community-based team.

* This role is ideal for a therapist with a passion for promoting independence in patients within their own homes.

Key Responsibilities:

* Deliver community-based assessments and interventions across the local area.

* Work with patients in their homes to improve functional independence and reduce reliance on domiciliary care.

* Collaborate with multidisciplinary teams to plan and implement effective care strategies.

* Maintain accurate documentation and ensure high standards of patient-centred care.

Requirements:

* HCPC registration as an Occupational Therapist.

* Proven experience in community settings.

* Excellent clinical reasoning and communication skills.

* Full UK driving licence and access to your own vehicle (essential).

* Able to work independently and manage a varied caseload
