Position: Care Coordinator

Location: East Cheshire, Macclesfield

Salary: £28,000 per annum

Employment Type: Permanent

We are seeking a highly motivated and compassionate Care Coordinator to join our domiciliary care provider based in East Cheshire, Macclesfield. Our company is dedicated to providing high-quality care to individuals in their own homes, and we are looking for a candidate who shares our passion for making a positive impact on the lives of others.

Responsibilities:

Coordinate and manage care services for clients, ensuring timely and effective delivery of care
Conduct initial assessments and develop care plans for new clients
Monitor and review care plans on a regular basis, making necessary adjustments as needed
Communicate with clients, their families, and care staff to ensure all care needs are being met
Conduct regular supervisions and appraisals for care staff
Assist with recruitment and training of new care staff
Maintain accurate and up-to-date records and documentation
Participate in on-call duties as requiredRequirements:

Proven experience in a care coordination role, preferably within domiciliary care
NVQ Level 3 in Health and Social Care or equivalent
Excellent communication and interpersonal skills
Strong organizational and time-management abilities
Ability to work independently and as part of a team
Knowledge of CQC regulations and standards
