Scheduling Coordinator

Job Description


Performs role under limited supervision. Responsible for coordinating the schedules of a multi-disciplinary team of associates, including nurses, physical therapists, occupational therapists, speech therapists, home health aides, and caregivers. Performs scheduling role promptly to obtain optimal patient/client outcomes, accurate visit management and efficient patient/client coverage. Serves as a liaison between field staff and patients/clients and their loved ones.


Coordinates scheduling of patient/client care needs- Schedules for private duty and adult foster care clients. Schedules for home health care multi-disciplinary team including, nurses, home health aides, physical therapists, occupational therapists, social workers, patient care technicians, speech therapists. Also responsible for coordinating the delivery of medical supplies.

Monitoring- Responsible for ensuring field staff's weekly caseload schedules are accurate and balanced in an attempt to avoid granting overtime.

Responding to telephone calls- Coordinates, resolves and/or directs incoming telephone calls. Communicates anticipated scheduled visits, makes appropriate adjustments as needed and communicates necessary changes in a timely manner. Follows up with hospitals when a patient transfers inpatient and completes a notice of transfer form to hospital case management.

Teamwork/Communication- Keeps clinical director informed of clinical and service line issues and coordination needs. Keeps vice president of operations informed of administrative type needs and issues. Keeps finance manager informed of associate trends related to open calls. Works collaboratively with other team members to assess coverage of all patient care needs. Handles phone requests calmly, effectively, and in a professional manner that supports customer service.

Other duties as assigned, including, but not limited to- Assists clinical director and vice president of operations with new associate orientation, covers on-call responsibilities, utilizes various computer applications to most efficiently perform role, participates in the successful operation of all three service lines by assisting in the intake and maintenance

Policy and Procedures- Assists in the implementation of policies and procedures in compliance with CHAP and regulatory standards. Maintains absolute confidentiality in accordance with the company confidentiality agreement.

Required Certifications, Registration or Licensure-

Valid State of Michigan's Driver's License

Current auto insurance

Minimum Knowledge and Education-

Required: High School Diploma or equivalent

Preferred: Associate's or Bachelor's degree

Minimum Work Experience-

Preferred: One year experience in the home health care setting.

Preferred: One year experience scheduling a health care setting.

Specific Knowledge, Skills and Abilities-

Computer skills- including basic to advanced level of utilizing Microsoft Word, Excel, and Outlook as well as utilizing the Internet for research purposes

Strong organizational skills and communication skills

Ability to maintain positive relationships with patients/clients, associates, and all other customers

Ability to work successfully in a stressful, fast-paced environment

Ability to work within a diverse team


Ability to stoop, bend and sit for long periods of time

Ability to use hands and wrists to perform repetitive motions on a keyboard
