About the Role
A rare opportunity has become available for a Social Worker to join the Wandsworth Hospital Team, based within the Wolfson Neuro-Rehabilitation Unit at Queen Mary's Hospital. This is a specialist setting supporting adults with complex physical, cognitive, and communication needs.
In this role, you will deliver a high-quality hospital discharge service that promotes independence, well-being, and positive outcomes for residents and their carers.
Key Responsibilities

Complete holistic assessments, manage risks, and make proportionate, evidence-based decisions.
Support safe and timely hospital discharges, including complex arrangements and housing applications.
Manage referrals, maintain patient flow, and act as a safeguarding enquiry officer.
Work closely with a multi-disciplinary team, attending weekly MDT meetings and Goal Planning sessions.
Liaise effectively with external agencies and local authorities, including supporting patients from other boroughs.
Contribute to a person-centred, strengths-based approach that empowers residents and families.About You
You will be a qualified, SWE-registered Social Worker with:

Strong post-qualifying experience within adults' social care.
Confidence working with individuals with physical, cognitive, and communication impairments.
Excellent communication, decision-making, and time management skills.
A proactive, solution-focused approach and commitment to high-quality practice.
Sound knowledge of relevant legislation, statutory duties, and safeguarding practice.What's on Offer
