Our client is on the lookout for a passionate and proactive Care Coordinator for this Domiciliary Home Care Branch ready to lead from the front, in delivering exceptional, person-centred care.

Role Overview

As Care Coordinator - you will be supporting carers, coordinating care and working hand-in-hand with the Branch Manager and integrated care services, to ensure smooth, compassionate service delivery. Primarily community-based, the position will include office-based work as the branch expands.

Key Responsibilities

Deliver hands-on care in the community
Create & implement individualised care plans
Organising your shifts, administering and recording medication
Liaising with health professionals and maintaining residents' computerised care plans
Assisting with induction, training and supervision of support staffRequirements

NVQ3 Health & Social Care or willing to work towards
Experience of supervising a care team
