Our client, Manchester Council, is seeking an Adults Social Worker to join their Hospital Discharge Team.

Responsibilities of this role:

  • Collaborate with multidisciplinary teams (MDTs) to coordinate timely and safe hospital discharges for adults with care and support needs.
  • Conduct Care Act 2014 assessments to identify ongoing care requirements and develop person-centred care plans.
  • Apply a strengths-based approach to promote independence and support recovery in line with the 2024 Hospital Discharge and Community Support Guidance.
  • Navigate and implement appropriate discharge pathways (0–3), ensuring continuity of care and appropriate community or residential placements.
  • Liaise with Care Transfer Hubs to manage complex discharges and secure interim or long-term care arrangements.
  • Safeguard vulnerable adults by identifying risks and ensuring appropriate interventions, including referrals to advocacy services where necessary.
  • Support unpaid carers and families, ensuring their involvement in discharge planning and access to carer assessments.
  • Maintain accurate case records and ensure compliance with the Mental Capacity Act 2005 and Human Rights Act 1998.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have minimum of 3 years post qualifying social work experience
