Care Coordinator - Complex Care - Littlehampton (Hybrid) - £28,000 - £32,000 - Monday to Friday

We are recruiting for a Care Coordinator to join a growing complex care provider based in Littlehampton.

The role offers hybrid working (1-2 days from home per week), with the remaining days based at the Littlehampton head office. This is a Monday to Friday role with no weekend requirements.

The Role In this Care Coordinator role, you will be responsible for the day-to-day coordination of care packages, ensuring safe, effective and person-centred service delivery.

You will manage rotas, coordinate staff and support the ongoing delivery of complex care packages within the community.

Key Responsibilities
Coordinate care packages, ensuring continuity and high standards of care
Manage staff rotas and ensure full coverage across all packages
Build and maintain strong relationships with service users, families and care staff
Support mobilisation and ongoing management of care packages
Work closely with clinical and operational teams to ensure safe service delivery
Manage day-to-day coordination and administrative duties in a fast-paced environment
Support communication, engagement and performance across the team
Requirements
Experience in a Care Coordinator role within domiciliary care is highly desirable
Experience or exposure to complex care is advantageous
Alternatively, strong experience in recruitment, scheduling or customer-facing roles will be considered
Highly organised with strong attention to detail
Excellent communication and relationship-building skills
IT literate, including Microsoft Office and care systems
Package
£28,000 - £32,000 salary
Hybrid working (1-2 days from home per week)
Monday to Friday only - no weekends
