Staff required for a well-established D2A (Discharge to Access) service across Taunton and West Somerset delivering a reablement and assessment service, working in conjunction with the NHS enabling patients to leave Hospital sooner with a level of support that they need to regain lost skills and abilities to live independently.Good background in care with experience in 'doing with and not doing for', must have a desire to provide high quality care.

You will need care experience to be considered for this role.

Full and part time posts available, 39 hour contracts working a 2 week rolling rota, including every other weekend. Shifts on a full-time rota are 7-2.30 and 2.30-10, part time to be discussed at interview stage. Drivers with own vehicles desirable, the option of a company vehicle for 39hr contracts subject to availability

Paid annual Leave, your travel time, and mileage is paid at 45p per mile in addition to your salary. We pay our staff half of their annual MOT test and provide them with complimentary breakdown cover plus many other benefits.

The role does require you to be a driver and be prepared to travel out of area when required, company car may be available for use, this is discussed at interview stage.

Purpose of role

To support a person to return to their home environment once discharged from an acute setting. The main purpose is to promote and support with independence, encouraging the person to return to their previous baseline where possible. Goals will be agreed and set on day of discharge with the individual, the key worker and the D2A team member. Goals are assessed daily to establish any ongoing needs and identify any other referrals to support with independent living. Documentation to be of a high standard to allow the assessment process to flow. The role requires the D2A team member to work closely with our partners, NHS, ASC, Village Agents, and other outside supporting agencies. Strong communication skills are essential for this role.
