Job Title: Adult Social Worker – Hospital Discharge TeamLocation: Hackney, LondonContract: Full-Time – 36 hours per week - Initiall 3 month contract - potential to be extendedRate: £32 per hour (Umbrella) £28 PAYEAbout the RoleWe are currently recruiting for an experienced Adult Social Worker to join the Hospital Discharge Team on behalf of Hackney Council. This role plays a key part in supporting safe and timely hospital discharges, ensuring individuals receive the appropriate care and support when returning to the community.You will work in a fast-paced, multidisciplinary hospital environment, collaborating with healthcare professionals, families, and community services to assess needs and coordinate effective discharge planning.Key Responsibilities

• Undertake strengths-based assessments to support safe hospital discharge
• Develop and implement person-centred care and support plans
• Work closely with NHS colleagues, discharge coordinators, and community services
• Facilitate timely discharges while ensuring appropriate care packages are in place
• Undertake safeguarding enquiries where required
• Apply legislation including the Care Act 2014 and Mental Capacity Act
• Maintain accurate case recordings and documentation in line with statutory requirements

Requirements

• Qualified Social Worker with Social Work England registration
• Experience working within adult social care, ideally within a hospital discharge or acute setting
• Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures
• Ability to work effectively in a fast-paced multidisciplinary environment
• Excellent assessment, communication, and decision-making skills
