Job Title: InReach/Discharge Pathway Co-ordinator - Home Treatment Team

Tripod Partners are looking for an Discharge Co-ordinator to join the Home Treatment Team in Guildford. This Band 6 role plays a key part in supporting patient flow, coordinating early discharges, and reducing delays for inpatients across the acute service line. The successful candidate will work closely with community and inpatient teams to ensure effective care transitions and timely discharges.

Key Responsibilities:

Identify patients suitable for early discharge or home treatment support.

Maintain an accurate log of all admissions and monitor discharge readiness.

*

Attend ward rounds and liaise with MDTs to support timely and safe discharges.

*

Coordinate with carers, CRHTTs, BedFlow, and stakeholders to facilitate discharge planning.

*

Monitor treatment plans and escalate concerns where criteria for discharge are not being addressed.

*

Provide daily updates on patient movement and discharge planning.

*

Support the admission and discharge process across community and inpatient services.

Position Details:
Location: Guildford
Duration: 3 months
Hours: Full-time, Monday to Friday, 08:00-16:00
Pay Rate: £27.67 per hour
Work Pattern: Monday to Friday

Requirements:

NMC registered Mental Health Nurse or relevant mental health qualification.

*

Experience working in discharge coordination or home treatment services.

*

Strong knowledge of inpatient and community mental health services.

*

Excellent communication, negotiation, and coordination skills.

*

Ability to travel between inpatient units across the region.
