We are looking for 3 Occupational Therapists to form part of the Homefirst Neighbourhood team (Adult Social Care)
This forms part of the home first hospitals service, focusing on the newly implemented discharge to assess process.
Must have their own transport -
Required to travel within South Oxfordshire to complete home assessments - minimum of once or twice a week but would fluctuate based on them being responsible for organising and managing her own calendar. We advise to book visits on the same days to reduce travel etc.
Bedside Assessments at the hospital required
Must be 2 years post qualified
Required to come into the office for occasional meetings but can complete paperwork at home.

Supporting people on their reablement journey
Care act assessments
Assessment skills
Organisation skills
Knowledge of appropriate legislation
Independent working and as part of a team.
