Responsibility to ensure safe, efficient, timely delivery of health care services from initial patient identification through procedure, hospitalization, and post discharge follow up. Participates in the development and delivery of education for staff regarding structural heart (SH) pathologies, procedures, and protocols. Serves as the primary clinical nurse coordinator for the care of structural heart patients.
a. Works collaboratively with the physician, nursing and support services to provide patient and family focused care.
b. Manages incoming referrals, screenings, patient flow, diagnostic testing, medical consultations and procedure schedules, pre and post procedural care. Coordinates, delegates, and schedules patient referrals and appointments for structural heart procedures.
c. Triages patients to appropriate structural heart therapeutic pathway by assisting with the coordination and clinical evaluation and collates and communicates laboratory and diagnostic data to appropriate providers.
d. Provides patient and family education.
e. Evaluates health and socioeconomic status and develops plan to meet patient needs.
f. Reviews, updates and communicates clinic status changes.
g. Participates in quality initiatives including STS Registry, data collection. Assists with the evaluation of the effectiveness of interventions in the structural heart program and identifies performance improvement opportunities.
h. Assists with the development and implementation of teaching tools for patients, families and MHMH clinical personnel.
i. Collaborates with SH providers to ensure efficient patient flow.
j. Maintains communication with referring providers and support staff, the structural heart team and other health care providers and health professionals to deliver optimal patient care.
k. Utilizes nursing process and nursing care standards to promote quality patient care and best practice treatment protocols.
l. Participates in SH rounds.
a. Participates in the development and delivery of education for staff regarding structural heart pathologies, procedures, and protocols.
b. Meets with the director of the structural heart disease program (title) at least weekly to discuss patients actively undergoing structural heart procedure evaluations.
c. Assists with the maintenance of spreadsheets with details of patient undergoing referral, evaluation, procedures and follow up.
d. Serves as a resource and positive proactive leader for the SH Program and Heart and Vascular Center.
e. Assists with the maintenance and communication materials for weekly structural heart team meetings.
f. Prepares the materials including power-point presentations for each structural heart case.
g. Assists with preparation of comprehensive reviews of program data from each of the structural heart related registry databases and volume projections and reviews program outcomes with the director of the structural heart team.
3. Performs other duties as required or assigned.
BSN with 5 years of nursing experience in Cardiology, Cardiothoracic Surgery, or Cath Lab or the equivalent of education and experience required.
Required Licensure/Certification Skills:
RN currently registered in the state of New Hampshire.
