Location: Kingshill, Kent ME19 4AE
Pay Rate: Up to £38 p/h

Job Description:

Join our Community Social Work Team (CSWT) in Kingshill, Kent, and help provide essential support to vulnerable individuals and families in the community.

Key Responsibilities:

* Conduct comprehensive assessments to determine care needs.

* Develop and implement care plans, ensuring they meet the needs of service users.

* Provide ongoing support to individuals, helping them achieve independence.

* Ensure that safeguarding and risk management procedures are followed.

* Work alongside healthcare professionals and agencies to ensure seamless service delivery.

Qualifications & Requirements:

* Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work)

* Essential: Registration with Social Work England (SWE).

* Previous experience in a similar role, with a strong understanding of community-based social work.

* Excellent interpersonal and communication skills.

* Strong time management and organizational skills.
