Qualified Adults Social Worker / Social Work Professional required to join the newly transformed Front Door service

You must have a social work qualification and full SWE registration as well as relevant experience to be considered
Pay rate £30phTo be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.

This is a great opportunity for Social Workers to be a part of an exciting transformation of the new Adult Front Door Service, alongside our existing award winning integrated neighbourhood Team. The Service support people at the earliest opportunity and maximise the effectiveness of early intervention and prevention with the aim to prevent or delay individuals from developing longer term care and support needs. This enables individuals to be healthy for as long as possible, self-managing long-term conditions.

The Adult Front Door Service is the first point of contact for individuals seeking health and well-being support. This fantastic Team will consist of Triage Assistants who will triage calls. We will have a Team of Social Workers, Support Planners and Triage & Prevention Assistants who will be linking in with a vast array of partner agencies to provide a person-centred response to requests for help and effectively support individuals to live well and as independently as possible.

The Management Team will consist of a Team Manager, two Advanced Practitioners and Two Team Leaders. This is an exciting time as you will be a part of service reviews and service developments through our transformation program and Corporate Plan.
