Due to increasing caseloads and the growing complexity of needs within our service, we are urgently seeking to expand capacity within our Early Help and Children with Disabilities team. This is a vital opportunity to join a dedicated and supportive team working to provide timely, effective interventions to children and families.

We are looking for a qualified and experienced Social Worker with a sound understanding of both Early Help frameworks and disability services. You will play a key role in ensuring that children and young people with disabilities receive the appropriate level of support at the right time.

• Support assessments and develop tailored care plans for children with disabilities
• Work closely with families and a range of partner agencies to deliver effective support
• Participate in safeguarding processes and attend multi-agency meetings
• Maintain accurate, high-quality case records in line with statutory requirements
