Care Coordinator (Worcester area) - in collaboration with the Clinical Care Manager and Registered Nurse, will act as primary point of contact by providing outreach, engagement, assessment and person centered treatment planning for individuals enrolled in the Central Community Health Partnership's Behavioral Health (CCHP/BH) Community Partners Program. Schedule for this position is predominantly Monday-Friday first shift. Schedules are designed to meet the needs of people served. As such, this position may require some evenings and an occasional weekend.
Qualifications include a Bachelor's degree in health-related field preferred. High School Diploma/GED and applicable experience in community-based behavioral health support programs as a community health worker, peer specialist, or recovery coach may be substituted for degree. Valid driver's license with a reliable and appropriately insured vehicle required. Experience working with adults with mental illness/substance use disorders, experience with accessing local resources, navigating health, behavioral health, and/or substance abuse treatment systems, and excellent communication, documentation, time management, and organizational skills strongly preferred.
